Job Summary
Health Home Care Manager responsibilities include engaging and enrolling clients into GRHHN health home programs, conducting needs assessments, developing and implementing comprehensive plans of care addressing medical, behavioral health, substance abuse and psychosocial needs, coordinating with care teams and providers, facilitating service team discussions, ensuring language access, and documenting activities for quality improvement and CMS/NYSDOH reporting. Requires driving, care coordination across medical and behavioral health services, crisis intervention, and ongoing patient and caregiver support within a Monday–Friday schedule.
Required Qualifications
- NYS Licensed Healthcare Professional – RN or SW preferred, otherwise Bachelor’s degree with minimum three years in Human Services or Associate’s Degree with five years’ related experience
- Minimum of 3 years of relevant clinical experience with Health Home’s target population
- Valid Driver’s License and proof of auto insurance
