Job Summary
Health Home Care Manager I provides client-centered care management and coordination for Health Home Program clients, developing individualized plans of care, delivering rehabilitative counseling, assisting families with environmental factors affecting health, coordinating with care team and providers, documenting case notes, utilizing EMR systems, and participating in case conferences and community outreach; role requires licensure/certification as an LPN or RN per NYS requirements and experience in behavioral health or substance use contexts.
Required Qualifications
- High School diploma/GED
- Associate’s or Bachelor's Degree in Health and Human Services with 1 to 3 years of experience working directly with persons with behavioral/mental health diagnosis, substance disorders, or linking individuals with community support resources
- Bachelor’s Degree, with 1-year related experience, in child and family studies, community mental health, counseling, education, nursing, OT, PT, psychology, recreation, recreation therapy, rehabilitation, SW, sociology, or speech and hearing
- NYS Licensure and current registration as an LPN or RN with 1 to 3 years of experience working directly with persons with behavioral/mental health diagnosis or substance disorders
