Job Summary
Provide supportive Health Home care management using care coordination processes for high-service users facing barriers to medical, mental health, social, and financial services. Create, manage, and coordinate enrollee care plans across all service continuums, including medical, behavioral, and social needs. Perform progressive community outreach for assigned cases, complete all necessary documentation within established timeframes, and generate weekly reports per AOT guidelines. Oversee cases involving mid-acuity and voluntary assisted outpatient treatment clients while providing 24/7 oversight as needed. Requires a valid NYS driver's license and QHP certification.
Required Qualifications
- Bachelor's Degree
- Valid NYS driver license as referenced in RRHS Fleet Program
- QHP - Qualified Health Professional
