Health Home Care Coordinator II

Coordinate client care by linking clients with health care and behavioral health treatment and other psychosocial services as dictated by the plan of care. Facilitate network-wide management of client care and collaborate with family members and service providers to ensure medical, behavioral, and psychosocial needs are met. Effectively communicate plans of care and progression through care management stages. Utilize health information technology to link services and participate in Primary Care team meetings. Prior discharge planning, counseling, home care, and substance abuse treatment experience is desirable. Strong organizational, management, leadership, and communication skills required to support coordinated, patient-centered care.
