Job Summary
Conduct outreach and eligibility screenings for Health Home services for Medicaid-eligible individuals, then collaborate to create comprehensive person-centered care plans addressing medical, behavioral, and social needs. Provide care management and health promotion education at community locations and enrollees' homes while assisting with appointment scheduling and monitoring service delivery. Offer referrals to family support and community resources as needed. This Full-Time role offers a Sign-On incentive of $1,500 and pays $21.50-$23.00 per hour.
Required Qualifications
- Bachelor's degree
- Bachelor's degree, preferably in Human Service, Forensic, Psychology, Public Health, or related concentration
- Two years' experience working with individuals with mental health, substance use or co-occurring disorders
