Job Summary
Investigate and assess social determinants of health in Manchester while collaborating with the Health Equity Coordinator on programs and partnerships. Research best practices, develop assessment tools, and prepare written and oral reports containing detailed findings and recommendations. Act as a subject-matter expert to facilitate program planning and deliver training on social determinants of health, barriers to care, and cultural diversity and inclusion. Prepare educational materials for community members and stakeholders to promote equitable positive health outcomes. Foster a culture centered in health and racial equity through work with internal and external partners.
Required Qualifications
- Bachelor's degree in public health, social work or a related field
- Valid Connecticut Driver's license

- Demonstrated capacity in understanding the social determinants of health and equity-centered program planning, both conceptually and in practice
- Experience in public health education, community outreach, social services, or other public health services
- Bilingual (English/Spanish)
