Health Educator/Outreach Worker II

Conduct structured educational sessions, group presentations, and formal event outreach to increase community awareness and utilization of health services. Solicit and assess patient interest, verify eligibility criteria, and provide referrals for medical, social, and emotional support. Identify site locations for outreach work, update curricula with pre- and post-tests, and maintain a resource directory of community programs and services. Develop accurate reports on program activities and patients served while assisting with orienting new staff to policies. Requires local travel, independent decision-making, and adherence to an 8-hour, five-day schedule.
