Job Summary
Conduct field and agency screening activities to identify individuals with unknown HIV/HCV/STI status and deliver confidential pre-test and post-test counseling. Perform rapid testing, properly label and transport specimens, and process samples for laboratory delivery. Provide HIV/HCV/STI awareness education sessions covering prevention, risk reduction, PEP, and PrEP. Ensure positive cases are linked to medical care and supportive services, including contact tracing, partner notification, and Ryan White care coordination. Maintain client case records, complete partner services and NYS provider report forms, and assist with program data entry and documentation. Submit all paperwork by designated deadlines and attend required staff meetings and trainings.
Required Qualifications
- 2+ years of Health Educator or comparable experience
- Experience as a communicable disease Pre-test/Post-test Counselor and Tester
- Experience facilitating group education workshops and providing one-on-one health counseling, support and information
- Strong interpersonal skills
- Ability to work collaboratively as part of a team
- Ability to travel to identified testing sites
- Flexibility with work schedule. Able to work nights and occasional weekends
- Basic Microsoft Office skills
- Minimum High School Diploma or High School Equivalency Diploma
