Job Summary
Health Coordinator (On Call) coordinates and monitors client physical and behavioral health services within residential treatment facilities, assists with intake and coordination of internal/external medical and behavioral health appointments (including telehealth), supports medication monitoring and documentation in eMAR/MAR, and provides direct client support within a structured residential environment. Responsibilities include using program-approved screening tools, applying Motivational Interviewing to engage clients, explaining program policies, conducting health education, observing client health status changes, assisting with access to healthcare services, and ensuring effective communication among clients, nursing staff, medical/behavioral health providers, pharmacy services, and program staff. Duties may involve coordinating after-hours intake and discharge processes, maintaining accurate documentation in electronic health records, ensuring policy-compliant medication handling, and responding to urgent health concerns with appropriate clinical staff or emergency services. The role requires strong organization, attention to detail, and the ability to support clients with complex medical, behavioral health, and substance use needs in a structured environment.
Required Qualifications
- High School Diploma or Equivalent
- One (1) year of experience working in a caregiving, human services, behavioral health, or social services setting, or experience supporting marginalized or vulnerable populations
- First Aid/CPR certification within 90 days of hire
- Proficient level of written/verbal communication, and efficiency with independent task management and ability to show attention to detail
- Basic computer skills and the ability to learn
- Demonstrated ability to maintain a professional, empathetic, and client-centered approach when working with individuals in recovery
Desired Qualifications
- Certified Nursing Assistant (preferred)
- Experience with electronic health records (EHR)
- Proficiency in Microsoft Office Suite, Outlook, and internet navigation
- MI (Motivational Interviewing) and client engagement techniques
- Bilingual abilities (if applicable)
