Job Summary
Health Coordinator (On Call) at Pioneer Human Services supporting residential facilities by coordinating client physical and behavioral health services, facilitating intake, scheduling and documenting medical/behavioral health appointments (including telehealth), assisting with medication monitoring and MAR/eMAR documentation, and providing direct client support in a structured environment. Responsibilities include coordinating care with nursing staff or Health & Resource Coordination managers, using Motivational Interviewing to engage clients, reviewing MAR for completeness, maintaining health service documentation in EHR, supervising safety and health education, and ensuring continuity of care across clinical teams in a Washington state residential setting.
Required Qualifications
- High School Diploma or Equivalent
- One (1) year of experience working in a caregiving, human services, behavioral health, or social services setting, or experience supporting marginalized or vulnerable populations
- Must apply for Agency Affiliated Counselor registration within 30 days of hire and obtain and maintain active registration within 90 days of hire in accordance with WAC 246-810-015; RCW 18.19.210
- First Aid/CPR certification within 90 days of hire
- Proficient level of written/verbal communication, and efficiency with independent task management and ability to show attention to detail
- Basic computer skills and the ability to learn
- Demonstrated ability to maintain a professional, empathetic, and client-centered approach when working with individuals in recovery
