Health Coordinator (Early Head Start)

Job Summary
Maintain accurate health records for enrolled children and prenatal clients while conducting home visits to assess their growth and development. Enter and monitor health data in KidsNet and ChildPlus, ensure services meet federal Early Head Start standards, and provide health education on preventative care. Make referrals to healthcare providers, follow up on services, and conduct outreach to community organizations for partnerships. Assist with preparing required reports and participate in case conferences to coordinate multidisciplinary care. Requires a Bachelor's degree in a health-related field, LPN or higher preferred, and CPR certification. Full-time role with 30 hours per week; access to a personal vehicle is mandatory.
Required Qualifications
- Bachelor's degree in health-related field
- A minimum of 1 year of experience working with infants, toddlers and families in an Early Childhood setting
- CPR and first aid certifications or be willing and able to become certified
- Employees must have access to their own vehicle, with valid insurance and a valid driver's license
- Ability to sit, stand, bend, walk and climb stairs
Desired Qualifications
- Licensed Practical Nurse (LPN) or higher
- Home visiting and or case management experience within a multidisciplinary team
