Health Coach - RN or RD

Health Coach position within the care team, partnering with the Diabetes Self-Management team to deliver diabetes and cardiometabolic self-management education and support. Responsibilities include coordinating communication among patients, families, and the multidisciplinary team; integrating pathophysiology and self-management knowledge into care; delivering individualized and group diabetes self-management education; developing patient-centered care plans; documenting in the EHR; ensuring confidentiality; producing reports; and contributing to quality improvement within a Patient Centered Medical Home model. Requires RN or BSN with relevant nursing experience, licensed in Arkansas, with willingness to pursue continuing education and collaborate across care teams; up to 50% travel.
