Job Summary
Coordinate the assessment, planning, implementation, and evaluation of Patient Centered Medical Home efforts for physician practices, including on-site reviews when requested. Monitor PCMH activities to assist with achieving NCQA or Joint Commission recognition, while communicating project goals, performance indicators, and timelines to internal and external clients. Implement periodic outreach presentations and educational programs to facilitate community participation in PCMH adoption, ensuring all data handling adheres to Louisiana state, federal protocols, and HIPAA compliance. This role requires extensive state travel and a valid driver's license, with a focus on coaching providers to adopt habits conducive to higher quality of life.
Required Qualifications
- Experience in medical practice or clinical setting
- Intermediate skill level with MS Office (Excel, Word and Outlook)
- Strong skills in business English and spelling
- Ability to maintain confidentiality
- Coaching/facilitating skills
- Strong oral communication skills
- Valid driver's license
- Extensive in state travel
- Ability to lift approximately 10-15 pounds with or without accommodation
