Health Coach - (CMA or LPN) - Greentree Medical - Full Time

Conduct population health outreach by identifying care gaps through analytic tools and patient registries, then contact patients to facilitate closing those gaps. Perform pre-visit planning via chart and data reviews, assist providers with clinical visits, and accurately perform clinical transcription under supervision. Coach patients toward self-management using motivational interviewing to set goals and reinforce care plans. Manage inventory, sterilize instruments, and perform in-house testing while ensuring all areas are stocked appropriately. Review registry and quality data to achieve population health goals and reduce avoidable emergency department visits. Requires current PA LPN licensure or Medical Assistant certification with two years of experience, CPR certification, and Act 34/33/73 background clearances. Full-time role at Allegheny Health Network in Pittsburgh.
