Conduct population health outreach by identifying care gaps through analytic tools and registries, then contact patients to facilitate closing those gaps. Perform pre-visit planning via chart reviews and assist providers during clinical visits by addressing care gaps and activating standing orders. Accurately perform clinical transcription and computerized provider order entry under supervision. Use motivational interviewing to coach patients on self-management, set goals, and reinforce clinical care plans. Clean and sterilize instruments, manage medical supply inventory, and perform in-house testing per established standards. Work hybrid on-site at 1070 Old National Pike, Fredericktown, PA. Requires LPN or Medical Assistant certification with two years of experience, CPR certification, and Act 34/33/73 criminal background clearances.
