Job Summary
Health Coach (CMA/LPN) at Allegheny Health Network in Erie, FT. Leads population health outreach, assists with clinical visits and care coordination, and provides self-management coaching to patients. Works with care teams to close gaps in care, supports chart/data review, documents elements of clinical visits under supervision, and assists with order entries. Demonstrates motivational interviewing to activate patients, reinforces the care plan, and participates in data review and quality initiatives. Maintains instrument sterilization and inventory, and adheres to HIPAA and privacy policies. Also performs in-house testing and supports remote access to population health data where applicable. Requires High School/GED or Medical Assistant/LPN credential and PA LPN licensure or equivalent, CPR, and required background clearances.
Required Qualifications
- Minimum High School / GED or Completion of a Medical Assistant Program or LPN License
- Current State of PA LPN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) OR Medical Assistant Certification
- 2 years of Medical Assistant experience
- CPR certification
- Act 34 Criminal Background Clearance
- Act 33 Child Abuse Clearance
- Act 73 FBI Fingerprinting Criminal Background Clearance
