Job Summary
Utilize evidence-based health coaching interventions to improve patient engagement and outcomes by guiding patients through provider-created treatment plans. Collaborate with physicians and nurse practitioners to develop integrated care plans targeting sleep, stress, nutrition, movement, and medication adherence. Conduct virtual synchronous sessions and asynchronous coaching to evaluate progress and adapt interventions using wearable data and patient-reported outcomes. Support personalized nutrition plans under a registered dietician's direction to reduce migraine attacks and autoimmune flares while following clinical workflows for documentation. Contribute to process improvements through regular feedback and assist with special projects as needed.
Required Qualifications
- Bachelor's degree is preferred
- Completion of a health coach training program that qualifies you to sit for the NBHWC Certification exam preferred
- Established experience working with patients with chronic diseases
- Demonstrated experience working within a medical model and/or an interdisciplinary team
- Demonstrated experience and comfortability working with patient data, including utilizing, and adapting to various technology platforms

- Receptive to receiving comprehensive training in the AndHealth integrative health coaching model and applying associated protocols in your patient care
- A strong sense of ownership, take initiative to make each patient's experience better than the last, and feel a strong responsibility for patient outcomes

Desired Qualifications
- Ideally looking for someone with a Registered dietitian or Certified Nutrition Specialist background
