Job Summary
Conduct 6-8 daily home visits and virtual sessions to develop wellness coaching relationships with patients, caregivers, and families. Collaborate with clinicians to create action plans, encourage healthy habits, and deliver education materials on nutrition, exercise, and disease management. Assist members in accessing community resources to address social determinants of health while maintaining HIPAA compliance. Coordinate care with vendors and team members to ensure optimal patient outcomes. Requires a high school diploma, minimum one year of health coaching experience, and proficiency in Microsoft Office. Schedule includes Monday through Friday, 8:00 AM to 5:00 PM Mountain Time, with mileage reimbursement provided.
Required Qualifications
- Minimum (1) year of experience as a wellness or health coach or educator
- High School Diploma or GED
- Associate's degree or two years additional experience in lieu of education
- Ability to communicate positively, professionally and effectively with others

- Solid knowledge of specific diseases, lifestyle-related topics, nutrition, and exercise
- Knowledge of and experience with applying the stages of change or a similar behavior change model
- Excellent interpersonal skills with the ability to foster trust and respect among customers and coworkers
- Able to work independently and remotely
- Able to work with protected health information and maintain absolute confidentiality and privacy in accordance with HIPAA regulations and AHP policies
- Proficient computer skills including Microsoft Office Excel, Word and PowerPoint
- Ability to work a flexible schedule
- Effective written and oral communication skills
- ability to establish and maintain a constructive relationship with diverse members, management, employees and vendors
- Ability to perform mathematical calculations and calculate simple statistics correctly
