Job Summary
Champion the integration of physical and behavioral health services through ongoing quality assurance and continuous improvement initiatives. Direct member enrollment, discharge, and transfer processes to ensure timely, accurate, and compliant transitions of care. Oversee the day-to-day operations of the Health Care Home, including the preparation and submission of all required monthly reports to the state. Analyze state population health reports to identify gaps in care and develop actionable strategies aligned with the agency mission. Prepare and provide oversight of the annual program budget while developing and maintaining departmental policies for compliance with state guidelines and CARF standards. Provide leadership oversight of staff, including performance management and professional development.
Required Qualifications
- Graduation from an accredited nursing program
- Registered Nurse (RN) license
- Master's Degree (MSN)
- Active, unrestricted Missouri RN license
- Ability to assess an individual's overall health using knowledge and skills grounded in psychology or related mental health field
- Minimum of five (5) years' clinical experience in direct service provision to adults and children in a health setting
- Minimum of two (2) years' clinical experience in a mental health setting
- Minimum of three (3) years' supervisory and management experience
- Valid driver's license
- Proof of insurance
- Reliable vehicle
- Strong communication skills
- Customer service skills
- Comfort with electronic health records
- Intermediate computer proficiency
- Use of Microsoft Office products
- Use of email
