Job Summary
The Health Care Coordinator will coordinate the delivery of participant medical and behavioral healthcare, providing education and direct clinical support to non-medical program staff. Responsibilities include assessing, planning, implementing, and evaluating care outcomes for individuals with complex medical conditions. The role demands excellent organizational and communication skills, a Bachelor's degree in Nursing, and a valid Pennsylvania driver's license. The position requires local travel and physical capabilities for direct participant intervention.
Required Qualifications
- Bachelor’s degree in Nursing
- Registered Nurse with experience in community nursing and/or developmental disabilities
- Maintain a valid Pennsylvania driver's license with an acceptable driver’s record
- Must be available for local travel

- Must be able to work as a team and independently
- Requires the ability to lift, carry, fold, kneel, reach, stoop, stack and bend
- May require crisis intervention in emergency situations
- Must be able to successfully administer CPR
- May be required to report to work during emergencies including inclement weather
