Health Care Case Manager

Job Summary
Act as a liaison between the SSVF Grantee and the VA or community medical clinic, providing case management, care coordination, and health education for Veterans with complex needs. Coordinate with the Veteran's primary care provider and the assigned interdisciplinary treatment team to address systems challenges and ensure equitable, Veteran-centered care. Collaborate with medical, nursing, and administrative specialists to identify and resolve barriers to accessing health care services or adhering to health care plans. Maintain sensitivity to individual needs regarding age, developmental requirements, and cultural factors while fulfilling administrative duties. Requires at least one year of experience in case management, coordination of community resources, and interdisciplinary collaboration, with a preference for experience working with veterans or vulnerable populations.
Required Qualifications
- Highschool diploma or GED
- Associate degree in the field of social sciences preferred
- Experience working with individuals with complex needs, particularly in behavioral health or care coordination
- Minimum of one (1) year of experience providing case management, coordinating community resources, and collaborating with interdisciplinary teams
- Current AZ Driver's License (valid and in good standing)
- Clean 39-month Motor Vehicle Record (MVR)
- Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
- Fingerprint Clearance Card
Desired Qualifications
- Associate degree in the field of social sciences
- Experience working with veterans or vulnerable populations
- Familiar and knowledgeable of available community resources
- Minimum of six months of recovery from substance use and/or mental health disorders
- Excellent judgment
- At least two years of experience in a health care or social services area of practice
- Sensitivity to all Veterans' individual needs concerning age, developmental requirements, and culturally related factors
