Job Summary
Coordinate access to health insurance, medications, and other health care needs for Health Assistance Program patients within the UVM Health Network, with emphasis on uninsured or underinsured individuals. Collaborate with healthcare professionals and pharmacy teams to ensure access to resources, affordable medications, and equipment, aiming to reduce inpatient hospitalizations and Emergency Department use. Serve as a key link between patients, healthcare teams, payers, and the community, and support insurance/benefits enrollment and care coordination efforts.
Required Qualifications
- Vermont Board of Pharmacy Technician license in good standing
- Minimum of 3-5 years experience in health care, case management, pharmacy or patient advocacy
