Job Summary
The Health and Wellness Navigator will deliver clinical/therapeutic services focusing on substance use disorders as part of an Assertive Community Treatment team. Responsibilities include providing clinical case management, developing treatment plans, conducting outreach, performing mental health and safety assessments, and acting as a team lead on substance use issues. Candidates must have 2 years of experience in substance recovery services and a strong understanding of various evidence-based practices, with excellent communication skills and ability to work collaboratively in a dynamic environment.
Required Qualifications
- 2 years of experience providing substance recovery services required.
- 2 years of field experience working with people with serious mental illness, co-occurring disorders and complex health issues and/or the chronically homeless population.
- Use of personal vehicle and proof of valid California Driver's License and current auto insurance, along with a clean DMV record.
Desired Qualifications
- CADACC I or II preferred or some SUD certification plus field experience in SUD setting.
- Proficiency in Microsoft Office programs, systems, and platforms.
Additional Requirements
- Some travel by car throughout the county region and surrounding areas.
