Job Summary
Health and Wellness Director (LPN/LVN or RN) responsible for the health, functional, and psycho-social well-being of residents in memory care, coordinating with the Executive Director to ensure high-quality care. Performs initial and ongoing resident assessments, initiates individualized service plans, and maintains the medication program. Oversees hiring, promotions, disciplinary actions, and termination of licensed nursing staff and care staff. Provides hands-on training, supervision, and staff development; communicates with residents' families; ensures compliance with federal/state/local requirements; maintains resident records and charting; participates in community marketing efforts.
Required Qualifications
- An LVN/LPN or RN nursing license in good standing.
- An associate degree or higher in nursing is preferred, or a combination of education and experience.
- Current state-required certifications, such as First Aid, CPR, and any other applicable credentials.
- At least four (4) years working in geriatric nursing, including experience with medication administration, delivery systems, and pharmacies.
- At least two (2) years in a supervisory/management role; experience with budgets, staff development, training, and scheduling.
