Job Summary
Health & Wellness Director (LPN) leading the clinical team in an Assisted Living community. Oversees all nursing services, provides clinical supervision and leadership while ensuring compliance with state, federal, and Americare standards. Collaborates with community leadership and interdisciplinary teams to guide assessments, care planning, medication management, and staff development. Maintains a safe, home-like environment, supports resident well-being, and ensures documentation accuracy. Roles may be known as LPN Director of Nursing, Assisted Living Nurse Manager, Memory Care Nurse Leader, Clinical Nurse Supervisor, or Wellness Director. Requires current state LPN license and proven nursing leadership experience in senior living settings; strong clinical knowledge, organizational and decision-making skills; excellent communication and teamwork abilities; commitment to resident quality of life and Americare values. Physical demands include lifting, pushing/pulling, and moving residents as needed.
Required Qualifications
- Current state Licensed Practical Nurse (LPN) license
- Proven nursing leadership experience, preferably in assisted living, memory care, or long-term care
- Strong clinical knowledge and leadership capability
- Ability to balance administrative responsibilities with hands-on clinical oversight
- Excellent communication and teamwork skills
- Dedication to supporting a high quality of life for residents

- Lift up to 25 lbs. and assist with resident transfers as needed
- Push, pull, and maneuver up to 100 lbs., including resident mobility assistance
