Health and Wellness Coordinator

Oversee day-to-day resident healthcare services, assist with assessments and care plan updates, and monitor condition changes to ensure medication management and documentation compliance. Partner with caregivers and medication aides to provide clinical guidance, help ensure appropriate staffing, and maintain accurate clinical records for state regulatory compliance. Monitor quality indicators, follow up on concerns, and collaborate with the Executive Director and Health & Wellness leadership to promote a resident-centered care culture.
