Job Summary
Health and Wellness Coach responsible for developing and maintaining wellness relationships, designing and delivering lifestyle change programs, conducting biometric screenings, creating care plans, and educating patients. Responsibilities include developing and distributing health education materials, coordinating with providers and staff, documenting progress in EMR and wellness portals, and supporting marketing of wellness programs. Requires a Bachelor’s degree or higher in a health-related field and strong communication skills; preferred RN or RD and 2-4 years of experience in a hospital/clinic setting. On-site role in West Lafayette, IN with a focus on evidence-based health promotion, weight management, stress reduction, smoking cessation, and chronic-condition education.
Required Qualifications
- Bachelor’s degree or higher in health promotion, health education, nursing, athletic training, nutrition, other health-related fields, or counseling
- Excellent verbal communication and public speaking skills to promote health and wellness
- Proficient with a computer and working knowledge of Word, Excel, Outlook, and PowerPoint
- Excellent customer service skills and problem-solving skills
- Ability to work well with a team and independently
- Able to maintain client confidentiality
