Health and Wellness Case Manager

Job Summary
Facilitate nonclinical case management and referrals for students of concern, serving as the primary wellness navigator to coordinate care among on-campus and community resources. Provide crisis response and de-escalation to students in distress while assisting families within FERPA guidelines. Develop prevention programming, wellness education initiatives, and outreach campaigns to promote holistic student wellbeing. Monitor student progress using the Advocate CARE system and collect data for university assessment needs. Act as a liaison to all university offices, manage health service messaging, and support students through academic and social concerns.
Required Qualifications
- Bachelor's degree in Social Work, Public Health, Health Education, Psychology, Human Services, Student Affairs, Nursing, or a related field
- One to three years of progressive experience in higher education, student support services, case management, healthcare navigation, wellness promotion, social services or related field
- Proficiency with Microsoft Office and general office technology
- Ability to learn institutional systems and databases
- Demonstrate commitment to and experience in student support and programming
- Ability to work within a diverse college environment
- Ability to communicate, problem solve, and manage multiple priorities
- Skilled at establishing rapport with students, staff, and internal and external partners
- Ability to create trust and maintain confidentiality
- Will need to be able to stand for extended periods
- Will need to be able to walk up/down multiple flights of stairs
