Health and Wellbeing Worker - Caboolture

Health Wellbeing Worker responsible for welcoming, empowering, and connecting Aboriginal and Torres Strait Islander clients to culturally responsive holistic primary health care. Acts as a navigator and connector, delivering health and social determinant information and education, supporting clients and families in goal setting and prioritisation of health and social needs, and implementing plans in collaboration with a multidisciplinary team to improve health outcomes. Essential qualifications include Criminal History Check, C Class Driver’s Licence (QLD), Blue Card, and CPR certificate; applicants must be Aboriginal or Torres Strait Islander. The role operates within Moreton ATSICHS clinics across Caboolture, Morayfield, Strathpine, Deception Bay, and Margate, contributing to an integrated team approach to Indigenous health and wellbeing.
