Job Summary
HCC Coder role supporting CMS risk adjustment and RAF scoring for a primary care team. Responsibilities include pre-visit and post-visit chart reviews to identify RAF/HCC conditions, post-claims reconciliation to ensure codes are captured, reviewing provider documentation to assign accurate ICD-10 and HCC codes, identifying documentation gaps and providing actionable feedback, tracking coding trends, and collaborating with Clinical Integrity, Outreach, and Provider Teams to maintain productivity and quality benchmarks while staying current with CMS risk adjustment guidelines.
Required Qualifications
- Minimum 2+ years of HCC/Risk Adjustment coding experience (required)
- Active coding credential preferred (CRC, CPC, CCS, or equivalent)
- Experience with chart audits and provider education
- Experience performing pre- and post-visit RAF reviews preferred
- Experience with EHR systems required
- Value-based care experience strongly preferred
