HCBS Team Lead, Intensive In-Home in Person County, NC

Job Summary
Provide individual and family therapy according to the consumer's Person-Centered Plan, complete clinical assessments and reassessments, and offer clinical guidance to IIH team members. Conduct weekly team meetings to monitor case progress, interventions, and overall team performance while ensuring compliance with intakes, authorizations, and documentation requirements. Track treatment outcomes, assist with transition and discharge planning, and collaborate with families, schools, and community partners. This role requires a Master's degree and current North Carolina licensure, with a focus on supporting children and adolescents with behavioral or mental health needs throughout Person County.
Required Qualifications
- Master's degree in Social Work, Counseling, Psychology, Marriage & Family Therapy, or a related human services field
- Current North Carolina Licensure or Provisional Licensure (LCSW, LCSWA, LCMHC, LCMHCA, LMFT, LMFTA, etc.)
- Minimum of one year of documented clinical experience with the population served
- Valid driver's license
- Reliable transportation
- Current auto insurance
- Ability to travel throughout Person County for home- and community-based services
- Strong communication skills
- Ability to maintain professional, ethical behavior
- Ability to engage with and support children, adolescents, and families
- Strong assessment, therapeutic, and crisis intervention skills
- Ability to lead, supervise, and coach team members
- Skilled in documentation, treatment planning, and progress monitoring
- Strong organizational and time-management skills
- Ability to analyze information and develop effective interventions
