Job Summary
Manage all HCBS referrals from initial inquiry through enrollment, developing relationships with Care Management Agencies, Health Homes, hospitals, and community organizations to generate and maintain referrals. Coordinate closely with Care Managers to facilitate timely enrollment into Children's HCBS, demonstrating expert knowledge of Serious Emotional Disturbance and Medically Fragile eligibility pathways. Coordinate and facilitate CFASS assessments, assist families in obtaining Children's Waiver services, and ensure all Medicaid and Managed Care documentation is complete and compliant. Track referrals, assessments, and enrollment timelines while maintaining accurate records in the electronic system. Serve as a knowledgeable resource for families throughout the intake process. Fully remote position with salary of $70,000–$80,000.
Required Qualifications
- Minimum 2 years of direct Children's HCBS intake experience
- Extensive knowledge of the New York State Children's HCBS Waiver
- Demonstrated experience navigating SED and Medically Fragile eligibility processes
- Proven success obtaining HCBS Respite, CFASS, and other Children's HCBS services for eligible children
- Established professional relationships with Care Management Agencies (CMAs) and other referral partners
- Strong understanding of Medicaid, Children's Health Homes, Managed Care Organizations, and Children's Waiver requirements
- Excellent communication, organization, and relationship-building skills
- Ability to independently manage a high-volume intake caseload
