Job Summary
The HCBA RN Intake Case Manager will perform intake assessments for HCBA waiver applicants, coordinating with physicians and service providers to identify, organize, and monitor required services. Responsibilities include conducting comprehensive health and psychosocial assessments, developing Plan of Treatment with the MSW and providers, tracking timelines, maintaining documentation and HIPAA compliance, and coordinating with the intake team to ensure service delivery. Travel within California is required (up to 2 visits per day; 4-6 client visits per week). Required RN license in California, CPR certification, strong computer skills, and effective written and verbal communication are essential. The role supports education, quality improvement activities, and regulatory compliance, including reporting incidents and maintaining licensure.
Required Qualifications
- Current, unsuspended California RN license
- CPR certification
- Proficient in computers
- Minimum two (2) years of experience in a home health organization, primary care clinic, or health facility (preferred)
- Minimum one (1) year experience in healthcare quality management (preferred)
- Report on pre-employment physical examination including TB screening or chest X-ray as required by agency policies
- Excellent judgment and knowledge of nursing practices
- Strong written and verbal communication in English
- Licensed driver with automobile and insurance in good standing
- Familiarity with Title 22 (CA)
- Willingness to travel up to 2 visits per day, 4-6 client visits per week
- Ability to complete assessments and coordinate services for HCBA Waiver participants
- Knowledge of HIPAA regulations and regulatory requirements
- Experience with documentation, charting, and case records
- Ability to develop goals and plans of treatment with multidisciplinary teams
- Ability to monitor timelines and compliance with agency policies
- Ability to maintain confidentiality and professional standards
- Ability to work with individuals with disabilities and provide accommodations as needed
