HCBA RN Intake Case Manager

RN Intake Case Manager responsible for completing assessments for applicant clients waiting to be enrolled in the HCBA waiver, including communication with physicians and identifying, organizing, and monitoring services needed. Will travel to client sites (up to 4-6 visits per week) and collaborate with the Social Worker on the intake team to develop plans of treatment, track and document interventions, and ensure timely completion of assessments in accordance with agency policies. Requires CA RN licensure, CPR, strong computer skills, and relevant healthcare experience. Physical duties include driving and field visits; mandatory HIPAA-compliant documentation, timely charting, and adherence to regulatory standards. The role involves education, counseling, advocacy, and coordination with providers to meet participants’ medical, psychosocial, and service needs, while maintaining confidentiality and participating in ongoing quality improvement activities.
