HBCT Clinical Care Advisor - BH/BH Crisis - Region 1

Conduct assessments to identify individual needs for youth in foster care and adoption assistance, then develop comprehensive care plans addressing physical and behavioral health objectives. Coordinate with multi-disciplinary teams to manage overall healthcare costs and ensure interventions are evidence-based and trauma-informed. Provide expert guidance on care decisions, promote least restrictive settings, and modify plans as needed to support member access to quality, cost-effective care. Evaluate health needs, identify applicable resources, and provide patient education, medication reconciliation, and follow-up care arrangements. Build meaningful partnerships with families while addressing cultural and linguistic needs, and work closely with various state agencies to coordinate social determinants of health. Perform case management telephonically and/or via home visits within the scope of licensure, with occasional office attendance for meetings or training.
