Job Summary
Assess program participant needs for outreach, counseling, education, and referrals using a harm reduction model. Develop strategies to support individuals using drugs/alcohol, people living with HIV/AIDS, and those at risk for violence. Provide service coordination, recovery-oriented support, and crisis intervention while maintaining person-centered service plans. Deliver training on harm reduction and HIV prevention to staff and participants. Maintain accurate documentation, advocate for quality external care, and assist with apartment safety and conflict resolution. Offer outreach to disengaged participants and provide holiday, evening, and weekend coverage as required.
Required Qualifications
- Bachelor's degree in Social Work or a related human services field or CASAC
- At least two years of experience working within a harm reduction model
- Thorough understanding of harm reduction
- Must be fingerprinted and cleared by the New York State Justice Center
- Skilled in articulating program goals
- Skilled in conflict mediation/negotiation and have an assertive approach to problem solving
- Ability to transfer knowledge and skills in areas such as, but not limited to: engagement, harm reduction, motivational interviewing, conflict mediation
- Demonstrated leadership skills and ability to work as part of a team
- Excellent oral and written communication skills
- Computer literate in Microsoft suite, skill sets in Word, Excel, and Outlook
- Be creative and flexible
- Show initiative and be responsible for follow through
- Ability to maintain confidential information, as related to position
Desired Qualifications
- Experience working with the homeless population preferred
