Job Summary
Provide intensive in-home harm reduction services and referral linkages for a small caseload of residents in Bridge housing throughout the Bronx, Manhattan, and Brooklyn. Collaborate with internal departments and external providers to coordinate care, while delivering harm reduction guidance and training to supportive housing staff. Promote clients' physical and mental health, safety, well-being, and recovery within the organization's 70-year history of helping New Yorkers with mental illness, homelessness, and incarceration. Bilingual (Spanish) preferred; computer literacy required for EHR documentation.
Required Qualifications
- Minimum of a high school diploma or equivalent (GED)
- Experience working within a harm reduction model or with high-risk populations
- Computer literacy required, including the ability to use the agency's electronic health record (EHR) system to document services
Desired Qualifications
- Bilingual (Spanish) preferred
