Harm Reduction Case Manager

Harm Reduction Case Manager responsibilities include conducting outreach to engage the target population, identifying community partners to engage PWUD, implementing harm reduction strategies to minimize health and safety risks, providing enrollment into the Syringe Exchange Program, facilitating brief assessments, reviewing assessments to identify community-based services for client linkage, and facilitating linkage to services through escort as necessary. The role operates Monday–Friday 8:30am–4:30pm and requires a high school diploma or GED, 1-3 years of relevant human services experience, and a valid driver’s license with access to a reliable vehicle.
