Job Summary
Implement habilitation plans by reviewing records, observing, and providing feedback to improve participants' skills. Support daily activities including personal hygiene, community outings, and volunteer training while ensuring participant safety and protection. Transport individuals to and from the Day Habilitation site, overseeing safe arrival and departure. Document site and community-based activities, report changes in physical condition to nurses and supervisors, and maintain equipment and supplies. Act as an advocate for participants' rights and ensure work-related safety procedures are followed.
Required Qualifications
- Valid and qualified NYS driver's license
Desired Qualifications
- H.S. or Equivalent preferred
- Min. one year working with the developmentally disabled population preferred
- Good Oral and written communication skills
- CPR/First Aid and defensive driving training preferred
