Habilitation Manager (I/DD services)

Job Summary
Act as internal case manager and advocate for clients in the Habilitation program within Erie, PA and surrounding areas. Participate in Person-Centered Planning processes and collaborate with clients, families, Case Managers, and Habilitation Techs to implement and evaluate individual care goals. Make regular client visits to monitor service implementation and evaluate progress against Person-Centered/Individual Support Plans. Carry out supervision of Habilitation Technicians according to policy and procedure and as mandated by state regulation. Participate in client specific orientations as directed by the Clinical Manager. Participate in an on-call rotation every 6 weeks, Monday-Monday.
Required Qualifications
- Bachelor degree
- 1 year experience working directly with I/DD population

- Demonstrated record of successfully taking on increased responsibility
- Ambition to grow and advance beyond current position
- Excellent communication and interpersonal skills
