Job Summary

As a full-time Habilitation Assistant, provide direct support and personal care to individuals, assisting with daily living activities (toileting, feeding, dressing, hygiene) and implementing individualized program plans and behavior support strategies under a Habilitation Specialist. Help individuals develop self-help, communication, and social skills to promote independence and community participation; supervise activities in classrooms, community outings, and recreational programs; ensure safety during all program activities and trips; transport individuals using agency vehicles; collect data and document progress toward program goals and treatment objectives; prepare concise progress notes, incident reports, and logs; participate in interdisciplinary team meetings and trainings; maintain a clean, safe environment; participate in recreational activities such as swimming or gym sessions; dispense prescribed medications as AMAP under RN supervision after certification/training; respond to crisis and medical emergencies, providing first aid when needed; uphold dignity and promote respect and safeguarding; may substitute in other program areas to ensure continuity of care; compensation of $20.17/hour after probation plus benefits; schedule Mon-Fri 9:00 am-4:30 pm.

Required Qualifications

  • High School Diploma or GED required.
  • AMAP approval at time of hire or within six months of date of hire.
  • Valid New York State Driver’s License, maintained in good standing
  • Ability to read, write, speak and understand English.
  • Strong interpersonal and communication skills for working effectively with the people we support, families, coworkers, and community partners.
  • Ability to respond appropriately to crisis situations and make sound decisions under pressure.
  • Commitment to maintaining confidentiality and safeguarding the rights of the people we support.
  • Ability to work independently and as part of a team.
  • Passion for ADAPT's mission and demonstrates a commitment to the non-profit disability sector.

Desired Qualifications

  • Six months of related experience is preferred but not required.
