Job Summary
Conduct comprehensive patient assessments, develop individualized treatment plans, and deliver personalized home-based physical therapy services. Monitor patient progress, adjust plans of care as appropriate, and educate patients and caregivers to support recovery and independence. Collaborate with physicians and interdisciplinary team members while utilizing innovative technology to efficiently document patient care. Manage your own flexible schedule while meeting Guaranteed Flex Pay role expectations of 10 visits per week. Practice independently in the community as part of an experienced interdisciplinary care team.
Required Qualifications
- Current New York State Physical Therapist License and Registration
- Valid driver's license
Desired Qualifications
- Minimum of one (1) year of Physical Therapy experience
- Successful completion of the VNS Health Rehab Intern program
