Job Summary
Conduct comprehensive patient assessments and develop individualized treatment plans for home-based physical therapy services. Deliver personalized rehabilitative therapy to patients recovering from surgery, illness, or accidents while monitoring progress and adjusting plans of care as needed. Educate patients and caregivers to support recovery and independence, collaborating with physicians and interdisciplinary team members. Manage your own flexible schedule focused on evaluations and revisit care, utilizing Ambient AI technology to reduce documentation time. Maintain a commitment of 10 visits per week with premium per-visit compensation in New York. Required Qualifications
- Current New York State Physical Therapist License and Registration
- Valid driver's license
