Job Summary
Conduct comprehensive patient assessments and develop individualized treatment plans for patients recovering from surgery, illness, or accident in their homes. Deliver personalized home-based physical therapy services while monitoring progress and adjusting plans of care as appropriate. Educate patients and caregivers to support recovery and independence, collaborating with physicians and an interdisciplinary care team. Manage your own flexible schedule focused on evaluations and revisit care, utilizing innovative Ambient AI technology to efficiently document patient care and reduce paperwork.
Required Qualifications
- Current New York State Physical Therapist License and Registration
- Valid driver's license
Desired Qualifications
- Minimum of one (1) year of Physical Therapy experience
- Successful completion of the VNS Health Rehab Intern program
