Group Hospice Nurse - RN

Job Summary
Visit patients in homes, long-term care facilities, and clinics across Lexington, West Columbia, Chapin, Newberry, and Batesburg-Leesville to evaluate needs, initiate telemedicine, and coordinate care with providers. Facilitate medical record transfers, complete consents, and enroll eligible patients into Care Management plans. Conduct cognitive screenings, Social Determinants of Health assessments, and medication reconciliations while documenting activities and updating care plans. Evaluate for home health, hospice, palliative services, RPM devices, and adaptive equipment. Reconcile prescribed and over-the-counter medications, provide post-discharge education, and ensure safe environments. Manage a full patient load with a 7-on/7-off or Pittman schedule, working 8-hour shifts Monday through Friday. Must have reliable transportation, a licensed driver's license, and valid vehicle insurance.
Required Qualifications
- Must be a licensed nurse
- License must be in good standing with appropriate board/issuer
- Must be able to work flexible hours
- Must be able to travel between offices, facilities, etc
- Must be a licensed driver
- Must have an automobile that is insured in accordance with state and/or organizational requirements
- Must have an automobile that is in good working order
Desired Qualifications
- Registered nurse preferred
- A minimum of three (3) years clinical experience preferred
- Experience in community settings preferred
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers
- Strong written and verbal skills
- Basic computer knowledge
- Ability to manage and demonstrate effective leadership skills
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances
- Ability to foster a cooperative work environment
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment
