Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams. Utilize an iPad clinical workflow to deliver preventive and diagnostic services to patients in their private residences. Perform virtual visits as needed and maintain flexibility to travel locally across licensed states. Engage one-on-one with members to answer health questions and guide them toward appropriate care without prescribing medication or altering existing treatment regimens.

- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: Grand Junction, Mesa, Delta, Montrose, Ouray, Pitkin, Gunnison, Chaffee area
