Assess, plan, implement, document, and evaluate outcomes for patients in their homes under physician direction. Utilize the nursing process to provide safe, effective, and patient-centered care while maintaining professional accountability. Evaluate the effectiveness of care provided by other direct caregivers and coordinate patient care collaboratively with interdisciplinary teams. Uphold patient rights, maintain confidentiality, and perform all duties in alignment with Mercy's mission, values, and Service Standards. Provide skilled, in-home care to patients of all ages across Benton and parts of Washington and Carroll counties.
