Job Summary
Provide direct inpatient care and daily rounding for hospice GIP and palliative patients, conducting comprehensive assessments, managing symptoms, and adjusting treatment plans as needed. Lead and document goals-of-care conversations with patients and families, then collaborate with hospitalists, specialists, case managers, and Shalom Hospice's IDG to ensure smooth transitions of care with discharge planners and hospice leadership. This full-time salaried role requires an active Tennessee NP license with prescriptive authority, board certification in Adult-Gerontology, Family Practice, or Palliative Care, and at least two years of inpatient palliative or hospice experience.
Required Qualifications
- Active Nurse Practitioner license in Tennessee with prescriptive authority
- Board Certification in Adult-Gerontology, Family Practice, or Palliative Care
- Minimum 2 years of inpatient palliative or hospice experience, preferably in a hospital setting
- Strong knowledge of hospice eligibility, GIP regulations, and Medicare documentation
- Outstanding communication skills and a commitment to compassionate, interdisciplinary care
