Job Summary
Complete hospice evaluations, starts of care, and revisits while assessing clinical status and eligibility for admission. Discuss clinical goals of care, generate referrals by building relationships with case managers, physicians, and APRNs, and manage admissions, discharges, and medication orders. Ensure appropriate consent forms, coding, and billing documentation for safe, efficient transfers of care. Hours are Monday through Friday daytime shifts at Milford and Bridgeport Hospitals. Required Qualifications
- Active RN License
- Minimum of 1-3 years of clinical experience in Hospice and/or Palliative Care
- Ability to assess patient clinical status and discuss care options for the state of illness
- Excellent planning, organization and presentation skills
- Proven ability to develop and implement a sales and marketing plan
- Excellent oral/written, communication and interpersonal skills
- Computer Skills and basic knowledge of iOS operating system
- This position requires an individual who is highly motivated, results-driven, and able to develop and build strong, lasting relationships
- The ideal candidate will have established healthcare contacts and be able to readily network in the community
Desired Qualifications
- Previous hospice sales experience preferred
- Bachelor's degree preferred
