Job Summary

GI Cancer Center Social Worker collaborates with the healthcare team to assess and plan interventions addressing the social, emotional, financial, and environmental challenges of patients and families. Responsibilities include psychosocial assessments, counseling, advocacy, education, resource linkage, coordination of family/team meetings, crisis intervention, and helping to overcome social and financial barriers to support adherence to treatment plans and optimize patient care and resource utilization. The role involves documenting professional recommendations, care coordination interventions, and treatment plans; supervising social work interns and contributing to training of staff; and educating fellows and residents on the impact of social factors on health outcomes. Requires a Master’s in Social Work, state licensure (LSW) with LISW within 5 years, and a minimum of two years of hospital/healthcare experience. The position operates in a hospital setting with standard work hours, located at the Cleveland Clinic Main Campus.

Required Qualifications

  • Master's degree in Social Work from a CSWE accredited program
  • Licensed Social Worker (LSW) required
  • Licensed Independent Social Worker (LISW) within 5 years of hire
  • Minimum of two years of hospital/healthcare experience
